Description

Reproclinic is an assisted reproduction clinic located in the Sant Antoni district of Barcelona and specializes in offering fertility treatments to both national and international patients. The founders of Reproclinic are Dr. Esther Velilla and Silvia Fernández.

Reproclinic's medical team is made up of 36 professionals from different fields: gynaecology, embryology, patient care, etc. The laboratory manager is Marta Asensio; while the medical director is Dr. Javier Ruiz.

Finally, Reproclinic has the ISO 9001:2015 quality certificate and several international recognitions.

History

Reproclinic was born with the intention of offering a service and totally personalized to all their foreign patients who come from countries like France, Italy, Germany, and Russia, among others.

They have all the necessary resources to ensure that the distance is not a problem and for patients to find various facilities during the process:

Instalations

Reproclinic is located in the central Barcelona neighbourhood of L'Esquerra de l'Eixample, very close to Plaza de Cataluña and Paseo de Gracia. This clinic has brand new and modern facilities.

Consultations are located in the main building of the clinic, located at Calle Comte Urgell N/48. In addition, this building has an area prepared to house in the near future the facilities of the sterile area, i.e. laboratory, operating room and recovery rooms.

As a result, the sterile area is now located in another building just across the street. Here is the IVF and andrology laboratory, the operating room and the patient recovery room spread over 100m2.

Below are all the areas of the clinic with the resources and technology they have available to perform fertility treatments.

Building and sourroundings

As we have mentioned before, Reproclinic has its main facilities in Comte d'Urgell street, number 48, right at street level and is characterized by two large windows decorated with corporate vinyls that welcome you to the premises. The Reproclinic building is on a corner and has two completely separate entrances: one for egg donors and the other for patients. In this way, separate circuits and confidentiality are always maintained, as well as the anonymity of the gamete donors.

The entire clinic occupies a space of about 1,500m2 distributed in a ground floor and a first floor. In addition, the entrance to the clinic is adapted for people with reduced mobility, as there are no steps and everything is at ground level.

Reproclinic's doors and hallways are wide enough to accommodate wheelchairs and there is an elevator to access the upper floor. Reproclinic's bathrooms are also adapted for people with reduced mobility.

Reception desk and waiting rooms

If you enter Reproclinic through the patient door, i.e. Calle Compte Urgell 48, you will find a reception desk on the right hand side. There are two people here who speak different languages.

Next to it, there is a large waiting room with individualized spaces to maintain the privacy of the patients. This whole area is decorated in wood and landscaping. This space has several armchairs and tables with plugs for charging electronic devices and wifi to make the wait more enjoyable.

Consultation rooms

Reproclinic has 5 medical offices, 4 for patients and one in the donor area. All of them have an examination room and a gynaecological table, although they do not have their own bathroom. Therefore, it is differentiated between the confidant area and the exploration area.

In the examination area there is a hand wash basin, the patient couch and a state-of-the-art Samsung ultrasound scanner offering high definition 2D and 3D images. In addition, there is a small space with a mirror where the patient can change in complete privacy.

Reproclinic has Dr. Javier Ruiz as medical director and andrologist of the centre, as well as 6 other gynaecologists specialising in assisted reproduction. All doctors are on call on weekends and holidays in case they have to perform any puncture or embryo transfer.

Surgery room

The sterile area of Reproclinic, where the operating room, recovery boxes and laboratory are located, is in a nearby building at 125 Sepúlveda Street.

The operating room is accessed after entering through a corridor that distributes the different sterile areas of the clinic. This area is directly connected through an automatic sliding door to the patient recovery unit and the IVF laboratory, allowing the transfer of patients and samples without losing the sterility of the environment. In addition, it is guaranteed that there is no alteration in the conditions and temperature of gamete and embryo culture.

This is a good sized room fully equipped with a patient stretcher, instrument tables, a monitor, a Logiq F6-GE ultrasound machine, a defibrillator, an anaesthesia machine, a crash cart and an ultra-purified air line. The entire sterile area has epoxy coated walls and rounded corners.

All these measures, together with the ultra-purified air, ensure that dirt does not accumulate on the walls of the clinic and sterile conditions can be maintained in the area. On the other hand, this operating room is also adapted to carry out major outpatient surgery, such as hysteroscopies, gynaecological biopsies, etc.

The staff present in all these interventions are the gynaecologist, the anaesthetist and the nurse.

Laboratory

Next to the operating room, Reproclinic has a large laboratory with 3 different areas:

  • In vitro fertilization
  • Andrology.
  • Cryopreservation.

In total, there are 3 embryologists working here who speak Spanish, Catalan and English. All of them are on duty on weekends and holidays.

On the other hand, the administrative area destined to the embryologists is located outside the laboratory, which provides greater sterility and cleanliness by not introducing papers or devices from the outside. In this way, it is possible to prepare medical records and organise the work without the need for the computers to be inside the laboratory.

Each of Reproclinic's laboratories, as well as the equipment available to them, are described below.

Andrology Laboratory

This is a small room where seminograms, semen capacitation, sperm selection and other tests related to male sperm are carried out.

For this purpose, this laboratory is equipped with a laminar flow hood, an inverted microscope, a centrifuge and an oven. Therefore, Reproclinic's andrology laboratory has everything necessary to carry out seminal procedures.

IVF Laboratory

The IVF laboratory is connected to the operating room, through an automatic door, and to the patient recovery area.

Light and temperature (22-26ºC) are regulated to maintain optimal conditions for embryo growth in the laboratory.

As in the operating room, the corners of the walls and the baseboards of the floor are rounded and the walls are coated with specialised paint for sterile areas, which prevents the accumulation of dirt and facilitates cleaning. It also has a positive pressure ventilation system and all the furniture in this laboratory is made of stainless steel.

The material and equipment available to carry out the treatments are as follows: 1 double laminar flow hood, to be able to work on both sides, 2 heated surfaces integrated in the hood, 6 heated blocks, 1 anti-vibration table, 2 binocular loupes, 2 Origio/Planer Benchtop incubators, 1 microinjector, 1 refrigerator and 4 automatic pipettes.

Recovery rooms

Reproclinic presents the recovery room within the sterile area, so that sterile conditions are not lost until the patient is recovered and ready to go home.

This room has 2 articulated beds and a stretcher to transfer patients to the operating room.

The privacy of the patient and her companions is maintained by separating the beds by a curtain. This layout allows the nurse to always be present in the control area of the recovery room. This ensures that patients who undergo surgery are always under surveillance.

In addition, after the ovarian puncture the Reproclinic staff offers water, juices, and something to eat according to the particular needs of each patient (biscuits with or without chocolate, toast, corn pancakes, nuts, etc.). Reproclinic also offers gluten-free products.

This room does not have a private toilet inside. However, a few meters away from the sterile area, the patient will be able to use the bathroom of the facility.

Sperm collection room

On the main floor of Reproclinic is the room for the collection of semen samples.

The resources available here are a television with a video catalogue and dimmable light that attenuates the environment.

These rooms also have a comfortable armchair, a sink and paper dispenser. In addition, this room has a buzzer so that the patient can alert the staff in charge of collecting the sample once he has finished.

Nursing room

In the nursing room, blood draws and the administration of certain drugs are carried out. Therefore, this room is equipped with the necessary elements to carry out this activity.

The nursing room is located in the main building, right next to the doctor's office.

It is a small room with a refrigerator to keep the reagents cold, an articulated stretcher for patients, utensils for safe blood collection and a small work area where the nurse explains to the patient how the medication can be administered during the cycle.

Reproclinic does not have biochemical analysis equipment, since patient samples are sent to an external service for analysis.

How is the first visit?

The first visit at Reproclinic is completely free of charge. This is an informative visit of about 45 minutes, where the doctor collects all the medical data necessary to create the patient's medical history. This first consultation can be in person or via video call.

The gynaecologist needs to know if the patient has undergone assisted reproduction treatments, assess the possible tests that the patient can provide and make an initial diagnostic orientation indicating whether further tests are necessary. Once this consultation is over, the patient will have an orientation of the most appropriate treatment based on her clinical case.

Generally, the first appointment is used to perform blood and catheter tests, as well as semen analysis and sperm freezing. The results appointment is usually made by video call to make it more convenient for patients and costs 80 euros. This amount is deducted from the treatment if it is carried out at Reproclinic.

Patients in treatment can have access once a month to group sessions of psychological accompaniment. These are carried out by the centre's psychologist if they are for patients who speak Spanish or Catalan. In the case of international patients who speak English or French, this psychological accompaniment is provided by Stephanie Toulemonde.

Foreign patient care

The philosophy of Reproclinic is to assign to each patient a team of 3 professionals who communicate openly with the patient 365 days a year:

  • Patient assistant: Takes care of advising and monitoring patient needs before or after the initial visit, be it in connection with treatment or travel (flight bookings, hotel reservations, etc.)
  • Medical assistant: Coordinates the entire treatment and is always ready to listen to the patient's needs. The assistant clarifies all questions within 12 hours via e-mail.
  • Doctor: Is responsible for the treatment and is also distinguished by his closeness to the patient. If necessary, patients can talk directly to the doctor in charge.

All 3 professionals speak the patient's native language and are available to assist the patient from beginning to end; i.e. each patient has his or her 3 contact persons at Reproclinic, and at no time are they replaced by others.

In addition, all necessary documents are provided in the patient's native language.

It is also noteworthy that Reproclinic has a support network of partner organizations in the patients' country of origin.

This allows patients to undergo tests, obtain prescriptions for their medicines, and/or carry out cycle monitoring with professionals in their country.

These measures ensure that the patient has to travel to Barcelona as little as possible while receiving high-quality treatment from her country of origin. This ensures that the patient is subjected to as little emotional and economic burden as possible.

This means that patients only have to travel to Reproclinic for procedures that require surgery and a laboratory, such as a follicle puncture and embryo transfer.

For IVF treatments, this procedure takes between 3-5 days, which means that patients must stay in Barcelona.

In addition, Reproclinic offers several guarantee programmes. On the one hand, Reproclinic has a guarantee programme for two blastocysts in an egg donation cycle.

Another programme that Reproclinic has is the Newborn Guarantee Programmes. These are reimbursement programmes that include 3 complete cycles, embryo cryopreservation and medication. In addition, all subsequent cycles with cryotransfers are included. However, to be included in the programme, the woman or couple must meet a number of requirements.

Finally, Reproclinic has a treatment financing programme for patients residing in Spain.

Gamete donation

Reproclinic has its own facilities for egg donors and takes special care that donors do not come into contact with patients, as the clinic offers a separate donor area from the patient area with its own separate entrance. Specifically, gamete donors will enter the clinic through a different door.

Just at the entrance, on the left hand side, you will find the Reproclinic reception for donors. In this area there is also a waiting room, a blood collection room and a gynaecological consultation room.

The clinical criteria to be followed by the clinic staff to assign a donor to each patient are based on phenotype, blood group and genotype. Reproclinic guarantees at all times the anonymity of its donors as stated in Law 14/2006 on Assisted Human Reproduction Techniques.

Other services

For successful fertility treatment, the intervention of other specialists may be necessary in addition to a gynecologist.

For this reason, Reproclinic disposes of the following specialist areas in order to offer its patients an even more comprehensive service:

All professionals are members of the Support Group both locally and in the patients' countries of origin.

For more complex cases, Reproclinic also works hand-in-hand with an andrologist, a geneticist, and a hematologist.

Last but not least, Reproclinic organizes a weekly medical committee in which all ongoing cases are discussed, negative pregnancy tests are analyzed, all patients are further monitored, etc.

Success rates

The Spanish Fertility Society is in charge of compiling the results of all the Spanish assisted reproduction clinics every year and making a statistical analysis. In fact, the SEF is the only scientific and independent organization of the clinics that collects and publishes these data in an objective manner.

Although our analysis of this clinic was carried out in 23rd September, 2021, we have to indicate that the last report published by the SEF shows the success rates obtained in the year 2020 (National Activity Register 2020-SEF Register).

Normally, there is a delay of about 2 years in the publication of these statistics due to the difficulty in data collection and the controls that are carried out. It should be noted that, to collect data from a gestation achieved in December 2017, we must wait until delivery, which will be approximately in September 2018.

It should also be taken into account that directly comparing pregnancy rates between different clinics can lead to misunderstandings because there are many factors that influence the results, such as the cause of sterility or the particular characteristics of each treatment.

The SEF does not provide an exact number of annual cycles that each center performs, but rather it does so in the form of ranges as follows: less than 100 annual cycles, between 100 and 500 annual cycles, or more than 500 annual cycles.

From inviTRA we consider that for a better interpretation of the results it would be good that the SEF provides more exact data or smaller ranges for the number of annual cycles.

Implantation rate per transfer

The probability that an embryo will implant in the woman's uterus after the transfer.

This success rate refers to the probability that a transferred embryo will nest in the woman's uterus. It is calculated as follows: number of gestational sacs observed on ultrasound divided by the number of embryos transferred.

The SEF shows this value in the form of an interval, since it is very complicated to provide an exact percentage due to the variability of factors that we have already discussed.

It should also be taken into account that the more assisted reproduction cycles the clinic performs, the narrower this interval will be and the better the interpretation.

According to Law 14/2006 on Assisted Human Reproduction Techniques, in Spain it is only permitted to transfer a maximum of 3 embryos at each attempt.

Even so, a higher percentage of single-embryo transfers is now considered a quality parameter for clinics, since although transfers of two or three embryos increase the rate of gestation, twin or multiple pregnancies have many risks for the mother and babies and must be avoided at all costs.

The recommendation of inviTRA is to transfer only 1 or 2 embryos whenever possible.
