Is it more serious to have obstructive or secretory azoospermia?

By Marta Barranquero Gómez B.Sc., M.Sc. (embryologist).
Last Update: 04/22/2025

Secretory azoospermia is more severe than obstructive azoospermia. In the case of secretory azoospermia, there is no sperm production. Therefore, it will be necessary to resort to a sperm donor to achieve pregnancy.

However, obstructive azoospermia is characterized by an obstruction of the seminal ducts, but there is sperm production. Males diagnosed with obstructive azoospermia can undergo a testicular biopsy to try to retrieve some sperm for use in IVF treatment to achieve pregnancy.
