Is it recommended to cancel the ovarian puncture if there is fever during the process?

It is difficult to give a generic answer to this question. We always have to individualise each case and not rely on general protocols in this type of situation. Each assisted reproduction treatment and clinical decision making must be based on medical criteria.

When we are carrying out a controlled ovarian stimulation treatment with a view to In Vitro Fertilisation (IVF), for which the patient will undergo surgery in the operating theatre in order to obtain the eggs that will be microinjected (ICSI) in the laboratory, we must always observe the signs and symptoms that our patient may present. Common short term side effects of stimulation are: abdominal bloating, breast tenderness, headache, nausea, etc. These are usually mild side effects of the medication. They are usually "bearable" and controllable and disappear when we perform the ovarian puncture.

If the patient presents with any other symptom that is not directly related to a side effect of the treatment medication itself, such as a fever above 37.5°C, we need to assess the context in depth. In other words, we must question the patient to find the "origin" or "focus" of this fever and, of course, examine her. We will ask about signs or symptoms accompanying the fever such as nausea, vomiting, diarrhoea, constipation, headache, sore throat, and so on. A general medical examination (auscultation, abdominal examination, examination of the lower limbs) and a complete blood test are highly recommended to obtain all the necessary information.

We must always assess the risk versus the benefit of continuing or not with the stimulation and carrying out the ovarian puncture, bearing in mind that the safety and health of the patient will always prevail over the results. The success of our treatments is to achieve pregnancy without forgetting to take care of the present and future health of our patients.
